Overview

Following last year’s huge success the 2nd Annual Reducing Hospital Readmissions & Discharge Planning Conference returns to Vancouver. The conference will bring together Medical Care Professionals, Hospital Administrators, Home Health Workers and Academics, to discuss strategies and initiatives to help reduce preventable readmissions and provide best practice patient care.

The 2 day agenda will present innovative approaches to patient centered care, successful practical front line case studies, opportunities to explore the importance of patient and community engagement, novel approaches to managing chronic medical conditions, and an exploration of ethical considerations for elderly patients.
